OKLAHOMA CITY - Gov. Mary Fallin has ordered a full review of Oklahoma's execution procedures following a botched execution Tuesday evening.

Death row inmates Clayton Lockett and Charles Warner were scheduled to be executed Tuesday evening at 6 p.m. and 8 p.m., respectively.

The Oklahoma Department of Corrections says Lockett's execution was halted following a "vein failure". Lockett later died of a heart attack.

“I have asked the Department of Corrections to conduct a full review of Oklahoma’s execution procedures to determine what happened and why during this evening’s execution of Clayton Derrell Lockett,” Fallin said.

Gov. Fallin has issued a 14-day stay of execution for Warner while the review is completed.
