Nurse Accidentally Disposes of a Kidney
TOLEDO, Ohio (AP) -- A report says an Ohio nurse who accidentally disposed of a kidney during a transplant didn't realize it was in cold slush she removed from the operating room and took down a hallway to a "dirty utility room."
The report was released Monday by health officials. It says the nurse said she was on break and didn't hear when a surgeon announced the kidney was in the protective slush during the Aug. 10 transplant at the University of Toledo Medical Center.
The report cites poor communication and oversight and insufficient policies as factors in the disposal of the kidney. It says the hospital has since enacted clearer policies about communication and the removal of contents from operating rooms.
The hospital hasn't said what happened to the intended kidney recipient. It hasn't returned a message left Monday seeking comment.

MOORE, Okla. (AP) -- Oklahoma highway officials say Interstate 35 in Moore has reopened in both directions.
But the Transportation Department said late Monday that travel in the Moore area was still discouraged.
The agency said first responders, utility crews and others performing official duties are working in the area and need clear lanes of travel.
The department says no other highways are closed due to the storms.
